i. There are 5 modes of payments available: JomPay/FPX/DDA/DCA/CDM
ii. Corporate client: Monthly payment based on the package chosen
USAGE TERM
1. First-time registration and membership activation is effective from the 1st day of the upcoming month
2. Members are supposed to make full payment before the 1st day of the upcoming treatment month for the continuation of membership.
3. This plan is sole to be used for treatment purposes only and members are advised to refrain from requesting any diagnostic or procedures which are not
related to the patient’s medical condition. Putracare management serves the right to audit any cases that raise a doubt of possible fraudulent practice and
if both the practitioner and members are found to have indulged in such malpractice, legal actions shall be taken which may include termination of
membership.
4. Exclusion criteria :
i. Vaccination/pre-order/supplements
ii. Screening tests
iii. Medical examination
iv. Medical Report
v. Purchasing medications without consultation
vi. Family members
vii. Specialist consultation & medication
viii. Cosmetics
ix. Wellness & Aesthetics
x. Elective & Preplanned Surgery
5. Client visitation records shall be audited every term i.e 6 months, and/if found to have an above 4 visits per month or equivalent of 24 visits over 6
months, client premium shall be reviewed by medical board. Based on advise of the medical board, client shall be subjected for:
i. Increment of premium, client will be notified on the amount prior to the action on increasing the fee
ii. Referral to secondary or tertiary centers ( centers that deemed necessary for follow-up)
iii. Account withdrawal if there is sufficient evidence of abuse, malingering, fraudulent activity is to be found
VERIFICATION
1. Clients have to mandatorily bring their ORIGINAL identification documents( I/C and PASSPORT) to clinic for verification
2. Clinic personnel has the right to reject the use of this plan if there is discrepancy of information in our database
3. If a client requires more than 1 visitation for similar problems for the treatment month :
i. He / She should bring along the old medications
ii. Once the medications are checked, they will be exchanged or upgraded once consulted by the medical professional
iii. Failure to produce old medications will result in penalty payment of RM 10.00
iv. The above practice is done to prevent abuse or misusage of the medications given to the patients
(*treatment month: the month that plan is active for treatment)
TREATMENT
1. Healthcare professionals will consult and treat in accordance with medical diagnosis
2. Treatment process that requires further investigation is to be solely decided by the health professionals only
3. Putracare Membership subscribers are entitled for medications up to 4 visitations per month. Any visitations exceeding that limit may need to co-pay for
the treatment cost. However we do not limit the consultation visits as you may seek our practitioner for unlimited times per month
4. Strict Identification Method by NRIC and Passport(Working Visa) and strict use of generic drugs, devices and outsourced services only
5. Advance Deposit for Consumer Account & Contractual with credit terms and limit for Corporate Account
6. Subscription is solely for five(5) CORE THERAPEUTIC healthcare purposes & Value added services for five(5) non-core preventive and aesthetic
healthcare
7. Visits within seven(7) days interval for similar conditions requires Subscriber to submit previous medication, failure of such shall render a Penalty Fee of
RM10.00 and exceeding four(4) visits per month shall render the subscriber co-payment RM10.00per subsequent visit if requiring medication dispensing,
strictly adhered to prevent abuse and misuse.
8. Account Churn or Default:
Suspension → 1month without any single payment
Withdrawn → Voluntary Discontinuation of Subscription, after which 6/12 window period/ applied in order to rejoin the membership
Review → Subjected to Medical Board [After which, Disease Exclusion/Account Exclusion shall be determined]
ID shall be transferred automatically to Non-member upon Suspension or Withdrawal or Review. Any re-transfer from Non-member to
Membership shall be subjected to six [6] months window period and Internal Medical Board review prior to resubscription.
9. PDPA, SOCSO & DOSH compliant.
10. Cost of Outsourcing Services shall be limited by strategic alliances and/or preparing the infrastructure to support the required outsourcing services.
OPERATIONAL HOURS AND AVAILABILITY
1. Clients can use this plan at ALL the Poliklinik PUTRACARE branches only
2. Poliklinik PUTRACARE operates EVERYDAY including on weekends and public holiday for 16 hours/day from 0730H till 2330H
3. PUTRACARE only operates 24 hours a day at central sub district branch. The central sub district shall accept cases from all the other branches within that
sub district. [Midnight Coverage Access (>23.30hrs)→RM20.00]
LAPSE
1. We strongly advise clients to make prompt payment in order to avoid disruption of service
2. If client intends to quit the membership, notice has to be given 30 days before the next treatment month
(*treatment month: the month that plan is active for treatment)
3. Clients who skip the scheduled payment will need to pay for the skipped month and for the treatment month in order to keep the account activated for
treatment.
TERMINATION
1. PUTRACARE membership has full authority to terminate the membership plan if there is evidence of:
i. Breach in agreement
ii. Misusing the plan for personal gain
iii. Forged identification
WITHDRAWAL/TRANSFER/SHARING
1. PUTRACARE membership is an individualized treatment plan, hence transfer or postponement of membership in case the member has deceased,
traveling overseas, prolong hospitalization, etc. will NOT be allowed under any circumstances. PUTRACARE membership also CANNOT be used by the
family members of the client as the treatment will be rendered to the person who is registered in the system only
2. The PUTRACARE has the right to amend the terms and conditions that deemed as necessary from time to time.
